Diabetes and COPD

In addition to transitioning home after a hospital stay, our nurses and pharmacists will work with you, your physician and family to develop a customized program so you'll be comfortable at home.

We will:

• Provide medication reminders, assistance, administration and reconciliation, as well as insulin and oxygen therapy at home
• Educate you on how to recognize early changes to decrease episodes of serious illness
• Assist with physician follow-ups (including transportation) and monitoring
• Help with daily home activities; such as bathing and dressing
